Form for capital markets services licensees to notify MAS when there is a change of its name or a change of principal place of business within 14 days from the change.

Financial institutions should complete and submit the form online via MAS-Tx . After submission, FIs may view a record of their submission under "All Transactions" in MAS-Tx.
