Form 18 - Notification to Change Particulars for a Representative

Form for licensed financial advisers and exempt financial advisers to notify MAS when there are changes to a representative's particulars such as changes in identification, business interests and contact details. The form is to be submitted no later than 14 days after the date of the change.

Form 18 is to be submitted online using the Corporations and Representatives System on MAS-Tx .

MAS-Tx is a centralised regulatory portal that consolidates transaction data from financial institutions across multiple MAS systems. It is operated by the Authority, which licensed financial advisers and exempt financial advisers under section 20(1)(a), (b), (c), (d) or (e) of the FAA must access to lodge the form.
