Form 4 - Notice of Change of Name and/or Change of Principal Place of Business
Form for licensed financial advisers to notify MAS when there is a change of its name or change of principal place of business. The Form is to be submitted no later than 14 days from the date of change.
Financial institutions should complete and submit the form online via MAS-Tx. After submission, FIs may view a record of their submission under "All Transactions" in MAS-Tx.
