Form for payment service providers to notify the Monetary Authority of Singapore of a permanent cessation of business.

Submissions must be made via the online form in MAS-Tx . Please note that a CorpPass ID is required. If the notification is being submitted using a CorpPass not belonging to the licensee, a scanned, signed copy of the Declaration document below must be attached to the notification.

You may refer to the specimen form below for preparations. This specimen form should not be used for submission to MAS.
