Form for licensed financial advisers and exempt financial advisers to submit information on their direct life insurance business. The form is to be submitted within 5 months from the end of each financial year.

Financial institutions should complete and submit the form online via MAS-Tx . After submission, FIs may view a record of their submission under "All Transactions" in MAS-Tx.
