Form F - Notice of Change of Particulars

Form for exempt insurance brokers to notify MAS when there are changes to their particulars such as changes in contact details or address or changes in shareholding or key officers. To be submitted no later than 14 days after the date of change.
Applies to: Exempt Insurance Broker

Financial institutions should complete and submit the form online via MAS-Tx . After submission, FIs may view a record of their submission under "All Transactions" in MAS-Tx.
