Form 5 - Notice of Cessation of Provision of Financial Advisory Service by a Licensed Financial Adviser (and Representative of such a Licensed Financial Adviser)

Form for licensed financial advisers to notify MAS when they cease any or all of their financial advisory services. The form is to be submitted within 14 days from the date of cessation.

Financial institutions should complete and submit the form online via MAS-Tx . After submission, FIs may view a record of their submission under "All Transactions" in MAS-Tx.
