Financial institutions (FIs) should complete and submit the form online via MAS-Tx. After submission, FIs may view a record of their submission under "All Transactions" in MAS-Tx.

Unless otherwise specified by the Authority in writing, member companies of the Singapore Exchange Limited (SGX) are not required to lodge separate Forms for quarterly returns through MAS-Tx to the Authority. The Authority will receive such member companies' quarterly submission from the SGX.
